Community Access
Southern Choices is proud to offer community access services to Toombs County and surrounding areas. We are also the only private run group to offer these services for your loved ones. We strive to provide community based activities designed to allow individuals to broaden their skills, grow and reach their full potential in their community.
Community Living Services
For individuals deemed eligible for Private Home Care Services, Southern Choices offers a wide range of services based on individual needs. These services are individually tailored supports that assist with the acquisition, retention, or improvement in skills related to an individuals continued residence in his or her family home.
Community Residential Services
Community Residential Services provides 24-hour residential supports to individuals with developmental disabilities. Individuals receiving community residential service reside in a Residential Home or a Host home environment. These services are only available to individuals age nineteen or older.
